All About Me Profile

Introducing your child to new teachers and caregivers can be challenging for both the parents and the child. Every child has unique interests, strengths, and needs. Parents/caregivers often need a simple and effective way to communicate that information to their child’s teacher at the start of the school year or a new PCA/babysitter anytime throughout the year. Family Achievement Foundation’s All About Me profile can help make this easier for everyone involved!

An “All About Me” profile is a great way to introduce your child to their teacher(s), therapists, other caregivers and/or any other professionals that will be working with them. It can be used when your child enters a new grade, moves to a new school, or is preparing for a big transition. The document is a great summary of your child’s needs, safety considerations and medical information.

This profile will allow you to drive the narrative for your child. It will include all the important things you think your child’s team should know and is a great way to open the lines of communication between you and your child’s team. This is a great tool for teachers and care providers to reference all year long! You can fill this out as the parent/caregiver or you and/or your child can fill out together.

What should I include in my summary?

Your child’s All About Me profile is a quick highlight of their strengths, needs, likes, reinforcers, etc. It allows a new person to gain a few important facts and get to know your child before meeting them. We have provided a basic template below.

Instructions:

Top Portion of Page 1:

Child’s Name
Age
Diagnosis or Diagnoses
*Be sure to add a photo*
List any medications and dosages
Allergies and/or dietary restrictions
Parent: include name, phone and email for each parent/caregiver
Check box if your child is speaking/verbal or non-speaking
- Include their speech abilities (AAC, Sign-Language, 1-2 word utterances etc..)
Other Caregivers can include PCA’s, Grandparents, Nannies etc..

About Me Section:

Add siblings, pets, where the child lives (City), a few favorite things or motivators etc..

I have difficulty with:

List any things they are having trouble with (ex: transitions between activities, going up and down stairs, loud classrooms)

Things that help:

List any motivators/helpful tools (ex: visual schedules, timers, calm voices)

I am really good at:

Include your child’s strengths (ex: being kind, making friends, singing songs, doing puzzles etc..)

Include things that may stress your child out (ex: loud noises, unexpected changes,

I can’t yet: What is your child working on, maybe in therapy or in school or at home (ex: ride a bike, use full sentences, climb a ladder, wipe after using the bathroom)

Things you need to know:

Include anything else about your child. Think about their favorite things like TV Show, Songs or Animals. Maybe your child loves trains or cats or MN Vikings. Emphasize any allergies or dietary restrictions again or Safety Concerns. You can also include things when they have Speech, Occupational or Physical Therapy, Swimming lessons or Music therapy.

In Case of Emergency:

Once saved as a PDF, you can digitally send the resource to your child’s team. I also like to provide a hard copy and will print the document on colored paper or card stock so that it stands out. That way it doesn’t get lost in the hustle and bustle of papers on a teacher’s desk.
